Request of a Dependent Youth to Remain in Care Past Age 18 Pursuant to 42 Pa. C.S.A. ยง 6302

  • This form can be used to document a youth's request to remain in care under Act 91 and can be used as a tool to explain and discuss the changes in the law with youth.

Sample Request Filed by a Youth to Resume Jurisdiction and Verification, With Instructions

  • This sample form can be used by youth who want to file a Motion to Resume Jurisdiction on their own. This document also includes section-by-section explanations and instructions so that youth can better under the form and the process for filing.
